Application of virtual reality in upper limb therapy in patients after a stroke

The theoretical part of the thesis deals with stroke from the medical and physiotherapeutic point of view with respect to movement deficit of the paretic upper limb. Specifically, the diagnosis of the disease, therapy and incidence including risk factors are mentioned. The practical part deals with 7 case reports of patients who underwent rehabilitation using virtual reality 2-3 times a week over the course of 8 weeks. The aim of these case studies is to determine the influence and evaluate the effectiveness of virtual reality therapy on functional deficit of the upper limb in different patients after stroke. The purpose is also to monitor practical aspects of therapy of specific patients after stroke, which will be taken into account in cooperation with the developers for future improvement of rehabilitation systems using VR. The Frenchay arm test, the modified Frenchay scale and the Visual Assessment Score of the Hand Functional Task are used to objectify the function of the upper limb. The evaluation is complemented by the quality of life questionnaires EQ-5D-3L and Short Form-36. This acquired non-traumatic brain damage represents the second most common non-traumatic cause of death in today's modern civilization, in the Czech Republic the incidence of this disease is around 300 cases/100,000 inhabitants. The consequences of a stroke are usually numerous, and one of the resulting disorders can be hemiparetic walking with spasticity. The theoretical part includes four main chapters. The first section is devoted to the description of a commonly occurring non-traumatic brain injury, namely the description of a stroke. The second chapter describes stereotype, kinesiology and gait analysis. Petra Reckziegelová Title: Case Study of Physiotherapeutic Treatment of a Post-Stroke Patient Objectives: The aim of this bachelor thesis is to process a case report of a post-stroke patient and to characterize this disease and present current treatment and diagnostic options. Methods: The theoretical part is based on the professional literature listed in the list of used literature and information sources. It contains current knowledge about stroke, stroke etiology, clinical presentation, diagnosis, therapy options and subsequent rehabilitation. The practical part is based on the materials I gained during continuous clinical practice at Kladno Regional Hospital, which took place from 9.1.2023 to 3.2.2023. The patient case report consists of anamnesis, initial examination, proposal of a short-term and long-term therapeutic plan, recording of the therapy course, final examination and evaluation of the therapy effect. Results: The main notable progress is an increased overal mobility of the patient and coping the self-care, resulting in improvement of self-sufficiency in activities of daily life. Effectivity of Combined Rhythmic Auditory Stimulation Rehabilitation Approach on Gait post Stroke Abstract Background: this review is assessing the effectiveness of combing forms of rhythmic auditory stimulation (a form of neurologic music therapy) to commonly used physiotherapeutic methods in the treatment of gait for hemiplegic/paresis stroke patients. Aim: to evaluate the extent in which Rhythmic Auditory Stimulation (RAS) combined therapy has a positive effect on gait for stroke patients, and whether RAS combined therapy is a more effective approach than conventional physiotherapeutic approaches. Methods: Databases Cochrane Central Register of Controlled Trials, Pubmed and Science Direct were searched. All databases were searched using a publication year range from 2009- 2022. Studies accepted were either Randomized Control Trials, Clinical Control Trails or Case Control Studies, evaluating RAS combined therapy versus conventional physiotherapeutic methods for Hemiplegic/paresis stroke patients. Possibilities of Bimanual Training of Upper Limb Coordination with People after Brain Damage with Hemiparesis

This bachelor thesis deals with the topic of selected bimanual training with people after brain damage with hemiparesis. The bachelor thesis is both theoretical and practical. In the theoretical part are presented the most common causes of brain damage that lead to hemiparesis. Then selected unimanual approaches are described, which focus mainly on the paretic upper limb in therapy. They are followed by bimanual approaches, which emphasize the coordination and involvement of both upper limbs. The unimanual ones are Constraint-induced movement therapy, Robotic Assisted Therapy, Functional Electrostimulation, and Proprioceptive Neuromuscular Facilitation. Bimanual approaches include the HABIT method, COMbined modified constraint induced movement therapy and bimanual intensive training, Mirror therapy, Bilateral isokinematic training, and Task-Oriented bilateral training. The practical part contains a created set of model bimanual activities, which include a total of 13 activities, from personal and instrumental everyday activities. Changes in lower limb muscle aktivity during walking in hemiparetics after application of botulinum toxin

This bachelor thesis deals with influence of botulinum toxin and for walking in hemiparetic patient (spastic hemiparesis after stroke). In the theoretical part, the problem of stroke an one of the very common diseases that cause central hemiparesis is approached from the beginning. Further, spasticity and its treatment are described here. The last part deals with the issue of hemiparetic gait and its rehabilitation. The main goal of the work was to record possible changes in gait parameters during the application of botulinum toxin A, which is applied every 3 months. The research involved 2 probands from regional spasticity center of the Hospital of Czech Budweis a.s., with central hemiparesis after stroke. They underwent entrance (2-3 weeks after application botulinum toxin) and exit examinations (1-2 weeks before application botulinum toxin), which consisted of kinesiological examination, examination of spastic paresis hip dysplasia according to the procedure J.M. Graciese, selected clinical gait tests (TUG, 2MWT), EMG examination and postural stability test (WBS). The whole measurements were performed to see, if any changes during the action of botulinum toxin could be objectifield, either clinically or by surface EMG. The obtained results were quite individual, bit despire a very small group of proband, it was possible to follow certain trends influence botulinum toxin. Surface electromyography has proven to be a suitable tool for monitoring muscle activity, but for future similar studies,will be need many more probands and repeat measurements to be include.
Physiotherapy for patients after stroke - problems of hemiparetic shoulder

Stroke is one of the most common causes of premature disability and is the second leading cause of death in all developed countries. It is a suddenly developing involvement of a certain district of brain tissue, which was caused by a disorder of its blood supply. One of the complications after a stroke is the painful shoulder syndrome occurring on the hemiparetic upper limb. It is pain in the shoulder area with simultaneous movement limitation. The theoretical part of the bachelor thesis deals with epidemiology, distribution, causes, diagnostics, complications and basic risk factors of stroke. Furthermore, the theoretical part presents the kinesiology of the shoulder and explains the painful shoulder syndrome at hemiparetics with subsequent treatment options. The main aim of this bachelor thesis is to map and summarize the problems of the hemiparetic arm, create a suitable rehabilitation plan and subsequently evaluate the effects of the selected physiotherapeutic treatment. This is a qualitative research, where the research group consists of 3 patients after a stroke with a hemiparetic shoulder. The case report consists of an initial entrance examination. This is followed by 8 therapies focused on the hemiparetic shoulder and finally the final exit examination. From the results is to deduce that two patients experienced some improvement after receiving physiotherapy in the shoulder area. The last patient was not able to perform an exit examination for health reasons. The work can serve as educational material for physiotherapists, professional public or students of physiotherapy who want to increase awareness of the issue.
Case Study of Physiotherapy Treatment of a Patient with Hemorrhagic Stroke

Title: Case Study of Physiotherapy Treatment of Patient with Hemorrhagic Stroke Aim: The aim of this thesis is to present a case study of a physiotherapy approach to a treatment of a patient with complications after a hemorrhagic stroke. The theoretical part of the thesis will focus on the pathology, risk factors and prevention. On the practical part I will present the case study with the patient's medical history, all the examinations done by me at the start and end of the treatments, day to day therapies and final results. Methods: The practical section is based on the case of a 51-year-old male, who was diagnosed with a hemorrhagic stroke in 6/7/2017. The study consists of physiotherapeutic approaches for an initial kinesiological examination on the first day, followed by eight therapy sessions lasting half an hour each, and on the last day a final kinesiological examination. All methods used were non-invasive. Results: The progress was notable during the ten sessions of therapy. The patient's gait has improved significantly in quality and quantity, the spasticity has improved and his ability to control the lower extremity and upper extremity. Title: Case Study of physiotherapy treatment of a patient after brain stroke Objective: Gain a theoretical knowledge about etiology, clinical symptoms, stages of disease, localization of haemorrhage and the following therapeutic methods. Method: The general part of the bachelor thesis contains theoretical knowledge about etiology, clinical symptoms, stages of disease, localization of haemorrhage and the following therapeutic methods. The practical part contains casuistry of the patient after the brain vascular stroke. Results: Light improvement of motivity and walk stability. During the therapy came to connection affected extremity to complex moves and symetric loading of lower extremities. Conclusion: Indication of physiotherapy of a patient after appeared highly active in the way of whole state health improvement.
